Donor Family Tribute Registration

I hereby give HonorBridge my permission to use photographs for the donor tribute slideshow, at special events, on the HonorBridge website, and in items published by HonorBridge or other organizations designated by HonorBridge for public/professional education on organ and tissue donation. The photos will be used only as authorized by HonorBridge or organizations designated by HonorBridge and always in a sensitive manner.
